Dr G,

I have received a couple of messages about Laserfit lenses. So I thought I would ask the questions here so everyone can read the answer

I was wondering if you could provide us with some details as to how you got to the stage of being able to make sclerals that are customized to the patients eye and then on top of that how you were able to incorporate wavefront correction? I know you were an early adopter of sclerals for the treatment of post lasik ectasia, did frustration with the trial lens spur you on to move contact lens fitting into the 21st century?

Even though I have addressed this question on my own website, I will repeat it here.
Like ever fitter today, I had a kit, or kits of different lenses.  In those days the selection wasn't as great because so few companies were engaged in this arena, but I must have accumulated about 6 different sets.  Each set attempted to address some shortcoming of the previous sets in terms of finding a "universal" fitting solution.  Then the company sold its patent - that lens is now available internationally - and I was faced with the prospect of starting all over again, learning another "method."  Since I had already established an international practice, the status quo was simply unacceptable.  Either I had to be free of the constraints I was forced to work under, or I had to stop accepting new patients.
Several things happened simultaneously.  One was the realization that eyes are sufficiently unique that a different approach was needed.  The second was my partnership with Truform Optics, a family-owned, but forward-thinking company engaged in the manufacture of custom RGP lenses.  The third thing was the acquisition of a new type of scanning device, called optical coherence tomography, which allowed me to scan the entire front of the eye in exquisite detail.  We quickly set about making scleral lenses from the scans, and then patented the method.  One of the unique features of the lenses made from these scans was the fact that they centered very well and did not rotate.  This allowed us to correct even the smallest amount of astigmatism.
However, was that enough?  Well no.  Even with the best scleral lenses with full astigmatic correction, patients were still experiencing distorted vision.  The glare, halos, and double vision persisted.  Granted it was much reduced, but still far from perfect.  The next step seemed to be wavefront optics.  But where to get such a thing?  We had the ideal platform, but nobody made such a thing and there was no road map to get there.  So, I just had to roll up my sleeves and get to work figuring it out, and that's just what happened.  Today I have one patent and another one in progress on my version of the technology.  Another thing I did was to invest in very sophisticated software to translate the complex mathematics into a lens surface, as well as to design the lens that contains it.
So, here we are 7 years later with the ability to design from scratch a lens that fits the shape of each individual eye, and with the optics designed from each unique optical fingerprint.  It is still a work in progress, but we are making strides every day.  We can now produce lenses that can virtually eliminate the ghosting and glare so common with keratoconus.  Visual acuity is usually improved by from one to several lines, and patients remark on how the quality of their vision has improved with these lenses.
